Making a Referral
To make a referral to our services, please complete the attached referral form and return it either by e-mail or post. Please ensure that you attach supporting documents such as current care/support plan, risk assessment, occupational health report and all other relevant information to support the application. Please pay attention to the referral criteria in the link below before completing the referral form.
Applications will be assessed on a first come first serve basis.

We aim to assess all applicants within 5 days of receiving the application. However assessment may be earlier depending on the level of need.
Protea will operate a waiting list of suitable applicants and provide regular updates on the progress of the application.
Protea Care will have the final decision on the suitability of applicants to the service.
